By checking “I AGREE” and clicking SUBMIT, you, as the Participant, agree on behalf of yourself and the Locations to be fully bound to the terms and conditions of the Master Participation Agreement identified herein with Sevaredent Sourcing Solutions, LLC, (the “Agreement”), effective as of today’s date (the “Effective Date”). You expressly authorize Sevaredent Sourcing Solutions, LLC, to populate your entity information within the recitals of the Agreement. Your acceptance today shall act as an electronic signature to the Agreement and shall be deemed as binding as original signatures. Sevaredent Sourcing Solutions, LLC, shall provide you with a final executed copy of the Agreement at the email address you provided at the time of your enrollment.
